This program is for children from birth to 10 years of age.

How much can be saved after enrolling the child under FGGP?

  1. 100% for Every 6 months consultation, Consultation for siblings (one time), Consultation for parents (one time)
  2. 20 % for Partial Orthodontic treatment with metal braces
  3. 15 % for Disking, Professional Fluoride Application, Fillings, Pus Drainage, Extraction
  4. 10 % for Oro-facial myotherapy, Pit and fissure sealants, Pulp Therapy, Crowns, Surgical Procedures, Additional appliances, Space maintainers, Orthodontic treatment with metal brackets, self-ligating brackets, ceramic brackets, lingual brackets & aligners and others.

How to enroll my child in the program?

  1. Fill up the registration form. Registration fee is will be utilized for registration and records maintenance for one year.
  2. You will have to show the Birth records/ Birth certificate/ Aadhar/ Unique Health ID of the child to assert the age of child.
  3. Once the registration is done, you will be given appointments for history taking, making records and follow-up visits.
  4. After registration, you can avail the benefits of the program.

 

What is the cost of the program?

One time Registration cost- ₹ 2500

Yearly maintenance cost- ₹ 100*

It is available in Smile Specialists Clinic during working hours.

Yes. Under the pre-launch offer, you can register for 1300 INR.

It is the ID provided by the Government of India. It is mentioned on the Covid Vaccine Certificate.

We would help you find your Unique Health ID or you can mention your Aadhar/ Pan number as well.

No. The form has to be filled and duly signed in person

Yes, you will be provided the digital copy of your records on submitting an application for the same without any additional cost. You will have to provide pen-drive for getting the digital copy.
