Terms and Conditions

Participants:

  1. Facial Growth Guidance Program is for children from birth to 10 years of age at the time of registration till 12 years of age or eruption of 2nd molar, whichever comes first.
  2. Under this program preventive and interceptive care will be provided at a reduced cost with an aim
    • To enable natural growth in children
    • To reduce the financial burden on parents
    • To promote preventive care in children
    • To intervene the developing malocclusion

Cost:

  1. One time Registration fee has to be paid at the time of Registration of child to the program.
  2. Renewal fee will be charged for Records maintenance after first year.
  3. Registration and renewal fee are non-refundable.
  4. Registration fee, renewal fee may vary for each year.
  5. For 2021, registration fee is 1300 INR for prelaunch period and 2500 INR after launch on 26th January, 2022.
  6. Renewal fee for 2022 is 100 INR.

Concessions:

  1. Concessions percentage on all the procedures will remain same for the duration of Program.
  2. Cost of procedures may vary for each year.
  3. Concession percentages for registrations in year 2021 will be as follows
    • 100% for Every 6 months consultation, Consultation for siblings (one time), Consultation for parents (one time)
    • 20 % for Partial Orthodontic treatment with metal braces
    • 15 % for Disking, Professional Fluoride Application, Fillings, Pus Drainage, Extraction
    • 10 % for Oro-facial myotherapy, Pit and fissure sealants, Pulp Therapy, Crowns, Surgical Procedures, Additional appliances, Space maintainers, Orthodontic treatment with metal brackets, self-ligating brackets, ceramic brackets, lingual brackets & aligners and others.

Visits:

  1. After registration, appointments will be given for history taking and follow up visits.
  2. Records viz. Photographs, Dental casts, Radiographs, Videos will be made at the time of registration and follow up visits as deemed necessary.
  3. Follow up every 6 months or as advised by the doctor is necessary.
  4. Reminder call will be made for the follow up 1 week and 1 day prior the appointment.
  5. If you wish, you can opt out for the reminder call.
  6. In case you cannot follow up, you need to inform the office regarding the same 1 week before the appointment date and set the next follow up date.
  7. Follow ups can be extended for 2 months or as advised by the participating doctor.

Treatment:

  1. You will be informed if any intervention is required.
  2. There will be certain suggestions made regarding procedures.
    • Compulsory Procedures viz. Pulp therapy, crown etc. It is advisable to go for the compulsory procedures. If not done, non-compliance will be marked and your child’s registration will be considered withdrawn.
    • Elective procedures viz. Pit and fissure sealants etc. You have a choice for the elective procedures. There will not be any implications on the status of compliance.

Compliance:

  1. You have to be compliant to the follow ups and treatment.
  2. At any stage you become non-complaint, your child’s registration will be considered withdrawn.
  3. Registration and renewal fees won’t be refunded.
  4. To continue the program, registration/ renewal will have to be done.

Support:

  1. Third party will be responsible for record maintenance, technical support etc.
  2. Data will be shared with third parties for the sole purpose of carrying the business.

Decisions:

  1. The decisions of Program Directors, Dr. Saumil Mogre and/or Dr. Shubhra Jain, will be considered final in all situations.
